What do I need to bring to my first appointment?
- Your patient details form completed if you have received your welcome pack in the mail or via email.
- Any X-ray/scan reports or films EVEN if they seem quite old.
- Your GP referral form (only required if you are an Chronic Disease Management (CDM) or Veteran Affairs DVA patient- refer notes below).
- List of any medications you may be on.
- Orthotics.
- Comfortable clothing.
- any other relevant information.
How long should I allow for a consultation?
For your first visit, please allow 45 minutes. Subsequent consultations will take about 30 minutes.
Do you have Hicaps facilities?
If you have a private health fund which covers osteopathy, our office is able to claim for you on the day of your appointment, which then only leaves the GAP amount payable by you.
Please Note: this is a different system to the CDM – Medicare claims and cannot be used in conjunction with EPC claims.
Can I claim my treatment on Medicare?
Only if your GP has referred you as an CDM patient. Your GP will have provided you with a form requesting 1 – 5 appointments that will be partly paid by Medicare.
The full amount of our schedule fee (refer enclosed fee schedule) is payable at each appointment and the amount of $53.80 will be rebated by Medicare. If you would like our office to claim this rebate for you on the day of your appointment you will also need to have your Medicare card and an Eftpos card that utilises a cheque or savings account for the rebate to be paid into. We are unable to bulk bill the Medicare treatments.
I am a DVA Patient, can I come in without a referral?
Unfortunately no. A referral from your GP is required to enable our office to claim payment from Medicare and will need to be renewed every 12 months or every 12 visits whichever comes first..
Do you treat Workcover or TAC Patients?
Yes we do. Please bring all relevant paperwork, case/claim numbers etc. Account is to be paid by patient in full. It is the patients duty to claim back the benefit personally via their employer or third party payer.
Appointment cancellation/change policy and charge.
Due to high demand for osteopath appointments with our osteopaths, we advise that a charge of $30.00 is payable for missed appointments or cancellations with less than 24 hours notice. A message on our answering service is more than adequate.
